For the Applicant Business:
- Current year-to-date profit and loss statement (less than 90 days old)
- Year-end company prepared profit and loss statement and balance sheet as of 12/31/2019 or your most recent fiscal year-end
- Year-end company federal tax return (including all schedules) for the most recent year end (may be 2018). If 2019 tax return has been extended, provide a copy of the extension document.
For Owners and Managers (principals owning 20% or more and general partners and managing members):
- Complete copies, including all schedules, of the most recent Federal tax return
For each Affiliate business (If any 20% or more owner of the applicant business owns more than 50% of another business, the other business is an affiliate. Parent businesses and subsidiaries are also affiliates.):
- Complete copies, including all schedules, of the most recent Federal tax return
